As a NP at Horizon, you will Provide initial psychiatric examinations or Medication Assisted Therapy assessments for individuals admitted to mental health, chemical dependency, and rehabilitation and recovery programs and provide follow up medication management services.
Participate in clinical case conference and case reviews to programs to provide consultation and guidance with difficult cases, assures communication in multiple forums with counseling staff to be updated on consumer progress and outstanding issues or concerns.
Prescribe psychotropic and/or addiction medication to ameliorate symptoms; reviews incoming physical exams to ensure if the information is current and accurate.
Communicate to primary counselor the need to make referrals for physical exams with consumer's primary care physician.
Ensure that all confidential and potentially sensitive information is processed, maintained, and utilized in a strictly confidential manner.
Review and sign all treatment plans for chemical dependency programs within 7 days of the multi-disciplinary team approvalas needed.
Document all services provided in the case record.
Must be literate with electronic health records.

MD or DO, PNP NP NY state license and board certified (minimally board eligible) in Psychiatry 1 year preferred, but not required in mental health or substance abuse.
Strong preference for individuals well versed in addiction medicine.
Location:
This position would be located at 291 Elm St, Buffalo NY.
You must be willing and able to travel between clinics if needed.
Hours:
This is a per-diem position that is scheduled Monday & Friday with various hours between 8:
00am-7:
00pm that can total to anywhere between 5-21 hours per week.
This will be discussed further in the interview process.
