Outreach and Education Manager

Pride Center of WNY The Outreach and Education Manager is responsible for oversight of the Methamphetamine Overdose Prevention Program, as well as the Lorena Borjas Transgender and Non-Binary Wellness and Equity Fund in coordination with the Executive Director.
This includes the coordination of related staff, grant deliverables, and continued process improvement focused on patient experience and outcomes.
As part of the essential functions of this role, the Outreach and Education Manager will:

Develops and implements work plans, goals, objectives, and ongoing strategy development toward assisting MSM who use methamphetamine and to engage in programming and treatment.

Coordinates the work and provides supervision of Case Manager.

Works with the Center for Behavioral Health to ensure that staff and community education is reflective of current workflows and processes.

Develops and maintains effective communication with staff from the Grant Management and Quality departments surrounding all reporting requirements, deliverables, outcomes, monitoring, and submission of data.
Ensures that internal and external reporting is timely, complete, and accurate.

Represents the agency in the community through participation in local workgroups and collaborations related to the scope of services offered by Pride Center of WNY.

Conducts outreach and education to promote the program(s) with community health and HIV/ STI testing providers, and with HIV positive and high-risk individuals, with a focus on MSM who use methamphetamine.

Ensures patient needs are identified and addressed and that health service delivery services are provided in an affirming manner.

Identifies emerging trends in the needs of the patient population and adjusts service delivery to meet these needs.

Maintains working relationships with referral sources including, but not limited to, NYSDOH, Erie County DOH, behavioral health, and other social service providers.

Develops social media and multimedia materials in conjunction with the marketing staff to enhance outreach and program promotion for all interventions.

Develops and maintains effective communication with program contract managers.

Assesses and monitors program compliance with funding requirements including data collection, monthly reports, and client records with Executive Director.

Cultivates and implements quality assurance activities including but not limited to weekly progress note/documentation review, chart reviews, AIRS data reports and client feedback surveys.

Develops ties with community agencies and community leaders for the distribution and promotion of program materials/events.

Assists in tracking of data and preparation of monthly and annual reports of activity to designated funders.

Participates in grant writing activities to support new and existing programming in collaboration with the Grants Management team with supervisor.

Maintains effective communication with the Grants Management team, program managers, and the Director of Quality surrounding all grant related reporting, deliverables, outcomes, monitoring, and data submissions.

Enters data timely and accurately to promote patient care delivery and participates in ongoing QI/QA activities and training to improve patient experience and increase data quality where needed.
Maintains and protects sensitive information in enterprise systems and uses/shares data in compliance with Information Governance and Security Policies.

Other duties as requested.
Qualified Candidate will have a Bachelors degree in Health and Social Services, Communications, Business, Public Relations/Marketing, or Education OR at least 3 years of experience developing/ delivering presentations and conducting outreach and networking events.
Candidate must be able to communicate clearly and professionally and demonstrate ability to work independently and exercise effective multitasking and time-management skills.
Ability to maintain Protected Health Information (PHI) in a confidential manner required.
Sensitivity toward cultural diversity, people living with HIV/AIDS and lifestyle and addiction issues essential.
Valid drivers license and access to a working vehicle is required.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
