Health Care Surveyor 2 (Nursing) - Syracuse

The Office of Aging and Long-Term Care is seeking Registered Nurses to join our multidisciplinary team of professionals dedicated to long term care quality. Duties include performing investigations and surveys of care delivered by licensed and regulated long term care providers and will play a key role in the informal appeals process during disputes. The Health Care Surveyor 2 (Nursing) will be responsible to work independently and with teams to ensure recipients of long- term care residential and health care services receive the necessary quality care and appropriate services to live safe and vibrant lives irrespective of the setting, including in a nursing home or adult care facility, or in residential homes within their community of choice. In addition, the incumbent will participate in the Division's Quality Assurance Process Improvement committee to identify efficiencies and opportunities for improvement in the survey and enforcement process. The incumbent will also perform other duties as assigned to meet the programmatic needs of the Office of Aging and Long-Term Care and the Department as a whole including emergency preparedness and response activities and providing input on the development of policies and proposals to improve the long- term care experience. Assigned program area and tasks may include but are not limited to the following:
Assigned program area and other duties are contingent on experience, preference, and then-current needs of the Office of Aging and Long Term Care. Home Care Surveillance:
Provides oversight of licensed home care services providers, limited home health care, certified home health, and hospice agencies. Through complaint investigation and surveillance practices, staff are expected to enter a community-based residence to perform the onsite component of surveillance. Nursing Home Surveillance:
Provides oversight of skilled nursing facilities and through a memorandum of understanding with the Office for People with Developmental Disabilities, intermediate care facilities for the developmentally disabled. Following the outline of the State Operations Manual for long term care surveyors, the team performs multidisciplinary recertification and focused inspections, as well as complaint investigations to preserve the integrity of care for the most vulnerable residents. Adult Care and Assisted Living Surveillance:
Provides oversight of adult care facilities including assisted living programs and assisted living residences. Through complaint investigation and surveillance practices, surveillance staff help to shape this wide sector of the long- term care continuum between residing in the community and a 24/7 skilled nursing facility. Complaint Resolution:
As assigned, this team accepts referrals from the complaint intake unit, reviews records, and performs necessary outreach to investigate and resolve a complaint about the care or services delivered by a covered entity (i.e., a nursing home, adult care facility, or home care or hospice provider).

Salary Range:
From $97029 to $106140 Annually

Qualifications:
possession of a license and current registration as a registered professional nurse in New York State; and two years of post-licensure, specialized professional administrative or professional clinical experience. Substitution:
a bachelor's degree in nursing may substitute for one year of post licensure clinical nursing experience; and a master's degree in nursing may substitute for two years of post-licensure clinical nursing experience. Preferred
Qualifications:
Clinical knowledge and experience with Long Term Care and aging population. Having provided direct patient care as a human services or clinical professional, and an ability to appreciate how the aging process affects outcomes. Flexibility and dedication are fundamental to the team's ability to address the needs of the program; Ability to actively participate on an interdisciplinary team, accept constructive criticism, and display constant professionalism as a representative of the Department; Good understanding and use with Microsoft Office Suite, particularly Work, Excel, Outlook; and Excellent written and oral communication skills. Able to demonstrate having achieved federal Surveyor Minimum Qualifications Test certification.Estimated Salary: $20 to $28 per hour based on qualifications.
